Derman Zorg is een professionele jonge organisatie die zich gespecialiseerd heeft in het leveren van multiculturele thuiszorg. Een instelling die zich verantwoordelijk voelt voor zorgverlening aan allochtonen en autochtonen. Derman Zorg is inmiddels uitgegroeid tot een organisatie met cliënten van diverse komaf. Wij bieden persoonlijke begeleiding, dagbesteding, activerend werk en huishoudelijke ondersteuning.

Wij zijn op zoek naar een ervaren, zelfstandige en gedreven:
**Ambulant begeleider **(part time / full time)

De ambulante begeleiding is er om de balans te herstellen en de eigen kracht van de cliënt te versterken. Begeleiding kan bestaan uit ondersteuning bij administratie, ondersteuning bij sociale contacten en begeleiding bieden bij enkelvoudige tot meervoudige/complexere hulpvragen. De cliënt vaardigheden aanleren om de zelfredzaamheid te bevorderen, zodat hij/zijzo zelfstandig mogelijk deel kan nemen aan de maatschappij. Ook zal je samenwerken met het wijkteam. Samen evalueer je de opgestelde doelen.
- Je taken_
- Je begeleidt toegewezen cliënt bij zijn of haar hulpvraag.
- Je observeert en voert hulpvraag verduidelijkende en ondersteunende gesprekken met de cliënt, en/of andere betrokkenen rondom de cliënt.
- Je stelt een begeleidingsplan op, evalueert en stelt bij.
- Je rapporteert in het digitale zorgdossier de voortgang en bespreekt bijzonderheden met het team.
- Je kunt overkoepelend organiseren, gebruik makend van het netwerk van de cliënt, mantelzorgers, vrijwilligers en voorliggende voorzieningen
- Je ondersteunt de cliënt actief in het onderhouden en uitbreiden van hun netwerk.
- Je hebt contact en zoekt samenwerking met wijkteams.
- Je werkt vanuit de participatie en eigen kracht.
- Je biedt emotionele ondersteuning (toont interesse en respect voor de cliënt).
- Je versterkt het zelfvertrouwen en de motivatie van de cliënt.
- Je profiel_
- Je hebt ervaring in ambulante begeleiding en hebt werkervaring met de doelgroep.
- Je bent ondernemend en initiatiefrijk.
- Je hebt inlevingsvermogen.
- Je beschikt over vaardigheden om gestructureerd te werken.
- Je kunt zowel zelfstandig als in teamverband werken.
- Je hebt oog voor mogelijkheden en benut de dagelijkse leermomenten om vaardigheden aan te leren.
- Je bent enthousiast, creatief en flexibel.
- Je bent communicatief vaardig en beheerst goede gespreksvaardigheden.
- Je bent in staat tot reflectie op eigen handelen.
- Je toont een grote gevoeligheid voor signalen van cliënten en reageert daar adequaat op.

**_Functie-eisen_**
- Afgeronde relevante mínimaal MBO niv.4 Opleiding (maatschappelijke zorg, sociaal werk en persoonlijk begeleider).
- Ervaring in ambulante begeleiding
- In het bezit van rijbewijs B.
