Zorgpartners has the mission to support people in staying safely and comfortably in their own home for as long as possible. By launching Full Package at Home (VPT), Zorgpartners expands its current long-term care services. We offer clients well-being, services, and care in their own homes.

ZorgPartner for clients at home
As a VPT nurse, you support and guide clients in their familiar home environment. This allows you to truly get to know your clients and provide them with the care that suits them best. Together with the client and any caregivers, you develop the care plan. You collaborate closely with various other parties involved such as home care, customer consultants, colleagues from other disciplines, GPs, etc.

In addition, you perform nursing tasks and have a monitoring and coordinating role regarding the well-being, care, and nursing of the client. You ensure that clients retain control over their own lives, take responsibility, and make their own decisions. In everything you do, the client's wishes take precedence.

Furthermore, as a nurse, you act as a point of contact for other colleagues in the VPT team and guide them on more complex practical issues.

• A completed MBO Nursing degree;
• Preferably (internship) experience as a nurse;
