We are looking for a dedicated and self-motivated Ambulant begeleider to join our team in Oost-Brabant.
Your key responsibilities:
- Provide personalized support to clients in their home environment
- Lead the ambulant group at Point O
- Develop and implement individualized care plans
- Support clients in their reintegration into society
- Build and maintain a strong support network for clients
- Offer emotional support to clients
- Identify and refer clients to relevant treatment options
- Collaborate with other healthcare professionals to provide comprehensive care
What we're looking for:
- A committed and self-sufficient individual
- The ability to plan and prioritize own tasks
- Familiarity with the 12-step Minnesota model (or willingness to learn)
- A valid driver's license and access to a vehicle
- Experience in the addiction treatment field or as an expert by experience is a plus
- High level of self-motivation and independence
- A minimum of MBO level 4 education in the social care sector or relevant work experience
