**About the Role**

We are a growing home care organization with over 11 years of experience in providing 24-hour care through live-in caregivers. Our mission is to provide the best possible care, enabling clients to live comfortably and safely in their own homes. As we continue to expand, we are looking for a motivated coordinator who wants to grow with us and shape the future of quality home care.

**Responsibilities**

As a Care Coordinator, you will be the central figure in managing and coordinating care for our clients. Your main tasks will include:

- **Care Coordination**: Plan and manage the logistics of caregiver arrivals, transitions, and departures.
- **Home Visits**: Conduct regular evaluations of care delivery and adjust plans as needed.
- **Communication Hub**: Act as the first point of contact for clients, families, caregivers, and partner organizations.
- **Family Engagement**: Lead conversations with families to ensure clear communication and shared decision-making.
- **Problem Solving**: Identify care-related concerns early and address them proactively.
- **Caregiver Support**: Provide coaching and advice to caregivers, helping them succeed in their roles.
- **Quality Improvement**: Continuously monitor and improve care quality and caregiver satisfaction.

**Requirements**:

- Degree in nursing or a comparable healthcare qualification
- Minimum Dutch language level of B1 (spoken and written)
- Fluent knowledge of English (spoken and written)
- Previous experience in care coordination or a similar role
- Experience with elderly care is welcome
- Excellent organizational and communication skills
- Flexible, solution-oriented, and calm under pressure
- Strong sense of responsibility and attention to detail
- Positive attitude and commitment to high-quality care
