1                Why a Paediatric HIV Advisory Panel?

Aidsfonds – Soa Aids Nederland, based in Amsterdam, is a Dutch organisation that also works internationally. At Aidsfonds - Soa Aids Nederland, we strive for a world where there are no longer any deaths from AIDS and where people enjoy good sexual health. A world in which everyone can love freely and without fear. We do this by working together with the people who are hit hardest by HIV, STIs, discrimination and exclusion. We strengthen their voices and support them with access to information, knowledge, and funding. For all that is love. Our three dream goals are: no more deaths from AIDS and no new HIV infections; sexual health and rights for all; and a cure available for everyone living with HIV.  For more information see: www.aidsfonds.org.

To have the most impact towards our strategic dream goals, Aidsfonds - Soa Aids Nederland involves communities through co-decision making on all organizational levels. For Aidsfonds - Soa Aids Nederland co-decision making, with meaningful involvement of people living with HIV, community activists and community experts and other members, directly contributes to innovative solutions for some of the most complex challenges facing the STI-, HIV and AIDS response.

1.1         About the Paediatric HIV Advisory Panel

In September 2023, we announced our inaugural Paediatric HIV Advisory Panel (PHAP) for the years 2023-2024. The panel's primary role is to advise Aidsfonds on paediatric HIV strategies, models, and funding mechanisms, as well as to collaborate in the decision-making process for funding, including Aidsfonds’ calls for proposals aiming to expanding the Kids to Care model to Zambia, Malawi, Tanzania, and Indonesia/West Papua under the paediatric HIV programs.

Five members have joined our first Paediatric HIV Advisory Panel bringing a wealth of experience in community-based paediatric HIV, capacity building of health workers on paediatric HIV, lived experiences as mothers living with HIV, strengthening caregivers' literacy on paediatric HIV treatment, and advocacy for the rights of children living with HIV.

The Paediatric HIV Advisory Panel members are committed to representing the voices of their communities by ensuring that the perspectives and needs of those they serve are genuinely considered in policy and decision-making processes. Together, they will work towards addressing issues related to the rights of children and young women living with HIV, striving to provide them with equal access to affordable, timely, acceptable, and quality HIV care services. The panel members come from different countries including Indonesia, Kenya, Namibia, Uganda, and Zimbabwe.

Our commitment to community-driven decision-making lies at the heart of our 2022-2025 strategy. We passionately believe that empowering the communities most affected is the key to impactful change. Community members possess exceptional insights into what works and what does not, thanks to their deep understanding of the intersections between issues and solutions. For this reason, we are looking to expand our PHAP to welcome two new members on board.

In 2023 the Paediatric HIV Advisory Panel (PHAP) was involved in the selection of four new country programmes, the development of our paediatric HIV Theory of Change model and the evaluation of the first six months of the PHAP. In 2024 the members joint an introduction meeting for all paediatric partner organisations of Aidsfonds. Furthermore, they will focus on the selection of a new partner in Cameroon, the adoption of the new Theory of Change, advise towards the pilot projects and be involved in the Paediatric HIV priorities as part of Aidsfonds new strategy (2026-2029).

For more details, please consult the Terms of Reference in Annex 1 and the Planning for 2024 in Annex 2.

2    Annex 1: Terms of Reference for the Paediatric HIV Advisory Panel

2.1         Introduction

To have the most impact towards our strategic dream goals, Aidsfonds - Soa Aids Nederland involves communities through co-decision making on all organizational levels. Aidsfonds is inviting expert members for the paediatric HIV Advisory Panel, as well as health care professionals/community health workers and community members, being (young) mothers with children living with HIV. The panel is established to advice Aidsfonds on paediatric HIV strategies, models, and funding mechanisms and to co-decide on funding e.g., for Aidsfonds calls for proposals for Paediatric HIV programmes.

For Aidsfonds - Soa Aids Nederland co-decision making with meaningful involvement of people living with HIV, community activists and community experts and other members, directly contributes to innovative solutions for some of the most complex challenges facing the STI-, HIV and AIDS response.

2.2         About Aidsfonds - Soa Aids Nederland

Aidsfonds Soa aids Nederland works in three roles: As an advocate, we raise our voice and join others to move governments, scientists, and funders to improve the rights and health of communities and scale up innovative solutions; As an expert, we work evidence-based and we built on the knowledge and experiences of communities, health professionals and the public to increase their knowledge and agency; As an involved fundraiser & funder, we support inclusive HIV cure research and innovative community solutions. For more information see: www.aidsfonds.org/.

2.3         About Paediatric HIV

Children (0-14) who acquired HIV have an elevated risk of morbidity and mortality. Half of infants with HIV infection will die before their second birthday if they do not receive treatment. This is unexceptionally high. According to UNAIDS 2023, an estimated 1.5 million children were worldwide living with HIV in 2022. In the same year, 130,000 children newly acquired HIV and 84,000 children died of AIDS. Children account for 13% of all AIDS-related deaths, even though only 4% of the total number of people living with HIV are children[1]. Many of these children are living in Aidsfonds’ focus countries (see Figure 1).

Despite the Sustainable Development Goal target to end AIDS as a public health threat by 2030 and the 95-95-95 targets[2] progress is stagnating. Progress on paediatric HIV remains behind because of several reasons such as limited funding for paediatric HIV interventions[3]. limited commitment of countries, and the failure to find and test children and their mothers to put them on treatment. An exceptionally high number of children living with HIV, over 40%, are not aware of their HIV status. Consequently, this contributes to a treatment gap. Adding to that is that it remains challenging to eliminate vertical transmission. The rate of transmission of HIV from a mother living with HIV to her child during pregnancy, labour, delivery or breastfeeding ranges from 15% to 45%. There are multiple gaps in preventing vertical transmission, namely, mothers not receiving antiretroviral therapy, mothers dropping out of care, or mothers getting infected during pregnancy or the breastfeeding period.

2.4         Aidsfonds Paediatric HIV programme

The Aidsfonds’ paediatric HIV programme (2015 – 2025) is based on an unwavering commitment to end AIDS-related deaths and reduce new HIV infections in children. Currently the programmes work in five countries[4] and seeks to expand to Malawi, Tanzania, Zambia and Indonesia/West Papua. We strive to eliminate vertical transmission[5] and ensuring all children living with HIV and their mothers should be able to live healthy and full lives.

2.4.1      The paediatric HIV programme:

l is impactful, between 2018 and 2021 a total of 336,237 children and mothers have been tested with HIV and 4,000 children and mothers were put on life saving anti-retroviral treatment.

l provides funding that is longer-term, flexible and supports community-based organisations directly. The paediatric HIV programme is funded by multiple donors, including ViiV Healthcare and private donations.

l is evidence-based, in 2015 we piloted community-based responses to paediatric HIV in Uganda. Based on the positive results and lessons we scaled the pilot to Mozambique, Nigeria, Zimbabwe and South Africa. Since then, we have developed the evidence-based Kids to Care model.

The Kids to Care model, is a community based approach, that effectively reduced new HIV infections and AIDS-related deaths by ensuring we find, test, and treat children and mothers living with HIV and support them to stay in care. The Kids to Care model:

l is community-based, rooted in community knowledge and comprises of four main objectives that mutually reinforce each other: (1) community-based HIV programmes linked to health facilities, (2) advocacy for improved paediatric HIV services, (3) build a knowledge base and (4) strong partnerships for cooperation, exchange and fundraising.

l puts communities at the centre because communities know their children. Community health workers are crucial in each stage of HIV care to ensure children get the care they need. Communities are involved through co-decision making to advice on Aidsfonds’ paediatric HIV strategies, models, and funding mechanisms and to co-decide on funding.

l is contextualized and sensitive to country specific needs, obstacles and challenges of children. Together with governments, partner organisations and the paediatric HIV advisory panel we work towards sustainable country specific solutions.

l contributes to Aidsfonds’ Strategy 2022-2025 'For All that is Love'.

2.4.2      Our four objectives:

  1. Aidsfonds together with partner organisations strengthens existing community-based responses and their collaboration with health facilities in paediatric HIV to find and support infants and children living with HIV, empower pregnant and lactating (young) women living with HIV and train health care workers, using the Kids to Care model as a best practice for paediatric HIV interventions.
  2. Aidsfonds and partner organisations advocate for governments and funders to invest in effective community based interventions and to provide access to and availability of HIV treatment and care including pDTG, EID. And for the integration of adolescent HIV-services into national SRHR and maternal health policies and services, including PMTCT, family-planning[6].
  3. Building knowledge base through research and data collection to generate evidence and to influence future programming, policies and funding at national, regional, and international levels of other organisations, donors, and governments.
  4.  Accelerate results by building strategic partnerships with national, international and global organisations including funders to amplify results and foster mutual learning (linking and learning) while securing vital resources to fuel community-driven solutions for for paediatric HIV. Notably, Aidsfonds works with GNP+, Global Alliance to End Aids in children and EGPAF, PATA, ViiV Health.

2.5         About the Paediatric HIV Advisory panel

Aidsfonds believes that a Paediatric HIV Advisory Panel serves as a voice for the communities, and that the panel’s expertise helps to create and fund more impactful projects. As mentioned above the panel has several roles:

Provide funding advice to Aidsfonds during the selection processes of proposals: In August 2023, a call for proposals is launched. EUR 1.600,000 is available to expand Aidsfonds’ Kids to Care model to Zambia, Malawi, Tanzania and Indonesia. Aidsfonds invites community-based organisations and non-governmental organisations, based in these four countries to apply through Aidsfonds’ online grant-making portal. The paediatric HIV advisory panel will discuss eligible proposals, provide feedback to the reviewed proposals and advise Aidsfonds on the funding decision. Consequently, selected applicants are invited for the due diligence process in preparation of a contract for the period 2024-2025. The due diligence and contract management will be handled by Aidsfonds.

Formulation of advice towards paediatric HIV strategies and models, funding mechanisms and evaluation of the Paediatric HIV programme. The panel will be invited by Aidsfonds paediatric team when paediatric HIV strategies and models are to be reviewed or evaluated, and to provide funding advice on e.g., for Aidsfonds calls for proposals for Paediatric HIV programmes.

Internal evaluation of work of the panel. When advisory processes and proposal selection processes are completed, there is a lot to learn from the process itself. The panel will engage on reflection and evaluation of the processes, to sharpen shared decision making and the processes to facilitate this. Lessons learnt will be documented and taken into consideration when formulating new advisory processes and for example adjusting panel governance structures.

The estimated time investment is expected to be between 5 to 7 days per year. More information about the regulations of the Paediatric HIV advisory panel can be found in section 7.

The Paediatric HIV Advisory Panel is considered as the primary vehicle through which the community shares local knowledge and lived experience, ensuring the work is guided by a robust understanding of the community and by relevant data that can lead to more effective projects. Below more information can be found on the establishment of the panel and its operating principles.

2.7.1      Time investment and Meetings

It is estimated that the membership of the Paediatric HIV Advisory Panel will take about five to seven days per year on average. The meetings are aligned with ongoing processes of strategy evaluation or development, funding allocations and other key processes within the Paediatric HIV team of Aidsfonds Soa Aids Nederland.

The quorum for meetings of the Paediatric HIV Advisory Panel in which advice is formulated, is 5 members. The members are not entitled to appoint delegates to attend meetings, or otherwise carry out their functions as members of the Paediatric HIV Advisory Panel in their place.

The Paediatric HIV Advisory Panel can meet on their own initiative or by a request from Aidsfonds - Soa Aids Nederland in order to address specific issues. For specific purposes, members of the Paediatric HIV Advisory Panel can be invited to the Paediatric HIV team meetings to provide input on topics relevant to the activities of the team.

During the first official meeting the Paediatric HIV Advisory Panel selects its chair and vice-chair for a period of two years. Each two-year period the (re)election of these positions takes place during the first annual meeting. The chair and the vice-chair will together identify and organise any need for support e.g., with understanding previous processes, understanding budgets/other background documents.

Aidsfonds Paediatric HIV project lead will take the position as secretary of the Paediatric HIV Advisory Panel to ensure good preparation of meetings and minute taking, availability of all relevant information and a connection with the organisation. Advice from the Advisory Panel will be duly formulated in writing and is directly given to the Paediatric HIV project lead of Aidsfonds Soa Aids Nederland. The advice is formulated and given based on consent with any minority voice included.
