About the Role

We are seeking an experienced and dynamic Social Worker to join our team at Midlands Partnership NHS Foundation Trust. As a Senior Social Care Practitioner, you will be responsible for providing high-quality social care services to adults with complex needs.

Key Responsibilities

  • Assess and manage complex cases, working closely with multi-disciplinary teams and partner agencies.
  • Develop and implement person-centred care plans, promoting independence and well-being.
  • Provide expert advice and guidance to colleagues and stakeholders, promoting excellent practice and professional development.

Requirements

  • Professional Social Work qualification and current registration with the relevant regulatory body.
  • Significant post-qualification experience of working with adults with complex needs and their carers.
  • Excellent communication and organisational skills, with a strong emphasis on teamwork and collaboration.
