About the Role

We are seeking a highly skilled and motivated Community Development Practitioner to join our Children's and Families Care Group. As a Public Health Practitioner - Health Visitor/School Nurse - Community Development Practitioner, you will play a key role in delivering the Healthy Child Programme to children, young people, and families across Staffordshire.

Main Responsibilities

  • Work closely with uni/multi-disciplinary teams/agencies to address a range of care/support issues across the spectrum of conditions.
  • Prevent unnecessary hospital admissions and improve the health and wellbeing of the population served.
  • Work alongside Team Leaders and Professional Lead to ensure synergy across localities.
  • Deliver services to a high standard that are accessible and demonstrate a commitment to equality and inclusion.
  • Be responsible for a defined population of service users in a variety of settings across the Partnership Trust.
  • Assess children and young people's care/support needs and develop, implement, and evaluate care/support packages.
  • Have supervisory, training, and leadership responsibilities where appropriate.
  • Work with the team leader to undertake delegated tasks in team management.
